Thank you for volunteering at St. Vincent's. To ensure the safety of our community, all volunteers must fill out this application.

Information on this form will be treated as strictly confidential.

After completing this application there are four more requirements:
1. Complete the online session for your Safe Environment Training. Instructions are at the bottom of this page, below the form.

2. Sign the code of conduct from the left sidebar of this page.
3. Please read and sign our Social Media & Technology Policy.
Steps 2 & 3, and the certificate from step 1, can be completed online in our volunteer paperwork portal. (Link in sidebar on desktop or at the top of the page on mobile.)

Training

- Must be updated Every 5 years.
1. Go to Albany.CMGconnect.org
2. Create an account or log in if you have an account already.
3. From your Dashboard select “Safe Environment for Lay People.”
4. Watch video and answer questions.
5. When you have completed the Video and Questions return to your dashboard
6. Under Safe Environment it will say “Completed” and “Print Certificate.”
7. Click "Print Certificate" and Download the file.
8. Email PDF of certificate to [email protected]
Social Media Use & Technology Policies
ONLINE:
1. Go to Albany.CMGconnect.org
2. Create an account or log in if you have an account already.
3. From your Dashboard select “2022 Policy Review Social Media Use & Technology Policies.”
4. Read policies and electronically sign.
5. Once completed return to your dashboard.
6. Under Social Media Use & Technology Policies it will say “Completed” and “Print Certificate.”
7. Click "Print Certificate" and Download the file.
8. Email PDF of certificate to [email protected]
IN PERSON:
Copies of the Social Media & Technology Policy are available in the parish parking lot office in a yellow folder. Please read over the policies, sign the form, and leave it in the folder.
